*The
Be a Hero vacation raffle will award four (4) vacation packages, which are subject to the following date, departure and other
restrictions:
• Three (3) nights Bahamas cruise on the Monarch of the Seas, departing Port
Canaveral on January 20, 2012, for two (2) persons. Includes all guest taxes and fees. Guests must have passports or
required documents. • Four (4) nights in New York City for two (2) persons. Includes
airfare from Orlando to New York and four (4) nights’ hotel and taxes for March 1- March 5, 2012. •
Seven (7) nights in Las Vegas - January 20 – 27, 2012, for two (2) persons. Package includes airfare
from West Palm Beach to Las Vegas, seven (7) nights at the MGM Grand Hotel and airport transfers. There is a mandatory
resort fee of $18.00 plus tax payable directly to the hotel by the guests, which is not included in the package. •
Ten (10) nights in Hawaii - April 17 – 27, 2012, for two (2) persons. Package includes airfare from Orlando
to Honolulu on April 17, transfer to hotel, five (5) nights in Waikiki at the Aqua Waikiki Wave Hotel, airfare to Maui
and five (5) nights at the Outrigger Royal Kahana Hotel. Package includes airfare, transfers, hotels and taxes. Each
person entering the raffle will select one (1) of the above vacation package prizes for which he or she will submit an entry.
No one may submit more than one entry or enter to win more than one of the four prizes. A separate drawing will be held
for each of the four prizes from among all entries received for that prize.
The vacation packages are valid for the
above specified dates and itineraries only. Winners are responsible for additional costs if they change dates or itinerary,
and any such changes must be arranged with It’s Vacation Time, Inc., by December 31, 2011. Prizes have no cash
value and are forfeited if not used on the dates, and in accordance with the terms, set forth above or as arranged with It’s
Vacation Time, Inc. Winners must call It’s Vacation Time, Inc., prior to December 31, 2011, at (772) 595-1550
to confirm travel arrangements. All terms and conditions governing the vacation packages are at the discretion of It’s
Vacation Time, Inc.
The Be a Hero vacation raffle is sponsored by Florida’s Blood Centers, Inc. (“FBC”),
whose principal place of business is 8669 Commodity Circle, Orlando, Florida 32819. Four (4) prizes will be presented,
in the form of the four vacation packages described above, which have been donated to FBC for this purpose by It’s Vacation
Time, Inc. The winner of each prize will be selected from among all entries delivered for that prize, in person, to
2311 South 35th Street, Ft. Pierce, Florida 34950, during the hours of 7:00 a.m. – 10:00 p.m. EST on November 9 and
10, 2011. The four prize winners will be selected by four separate drawings to be held on November 10, 2011, at 2311
South 35th Street, Ft. Pierce, Florida 34950, at 10:15 p.m. EST; and the winners will be notified by 5:00 p.m. EST on Monday,
November 14, 2011. Only one entry per person, for only one of the four prizes, will be permitted, and persons must be
at least 18 years old to enter. Employees of FBC and members of their families are not eligible. No purchase or
donation is necessary to enter; however, FBC suggests that a blood donation be made at the Be a Hero Blood Drive at 2311 South
35th Street, Ft. Pierce, Florida 34950, on November 9 or 10 in order to participate. FBC is not affiliated with It’s
Vacation Time, Inc., and all terms and conditions governing the vacation packages are at the discretion of It’s Vacation
Time, Inc. Prize winners are responsible for any state or federal income taxes that may be imposed as a result of winning
a prize. Blood donors must have a photo ID.
